Skilled Nursing & Therapy

Comprehensive in-home clinical care designed to help patients recover safely, manage chronic conditions, and avoid unnecessary hospital visits. Services include skilled nursing, medication management, physical therapy, occupational therapy, speech therapy, disease education, and care coordination tailored to each patient’s needs.

Personal Assistance Services (PAS)

Non-medical support services that help patients maintain independence and quality of life at home. Assistance may include bathing, grooming, dressing, meal preparation, mobility support, companionship, light housekeeping, and daily living activities. 

IV Therapy & First Dose Education

Professional administration and monitoring of intravenous therapies in the comfort of the patient’s home. The program also provides first-dose medication education, patient monitoring, side-effect management, and caregiver instruction to ensure safe and effective treatment.

Wound Care & Advanced Foot Care

Specialized wound management focused on promoting healing, preventing complications, and reducing hospitalizations. Services include wound assessments, dressing changes, infection prevention, diabetic wound care, pressure injury management, and advanced foot care for high-risk patients.

Continence Support Program (PureWick Assist)

A supportive continence care program designed to improve comfort, dignity, and skin protection for patients experiencing urinary incontinence. The program includes education, device assistance, skin integrity monitoring, and caregiver support using the PureWick system when appropriate.

CLIA Waived Laboratory

Convenient point-of-care laboratory testing that allows for faster clinical decisions and improved patient monitoring. Services may include basic diagnostic testing, infection screening, glucose monitoring, urinalysis, and other CLIA-waived tests performed under compliant healthcare standards.
